Prep a short question list before a doctor visit

The problem: You finally get the appointment, the doctor has ten minutes, and the moment you leave you remember the thing you actually wanted to ask. Symptoms, dates, and medicines are all in your head, and none of it comes out in order.

Do this

  1. Jot down what’s going on in plain words: what you feel, since when, what makes it better or worse, and what you take. Leave out your name, birth date, and insurance numbers.
  2. Fill in the brackets, paste the prompt, and add your notes at the bottom.
  3. Read the list, fix anything that’s wrong, then print it or keep it on your phone and hand it to the doctor at the start of the visit.

Copy-paste

Help me get ready for a doctor's appointment. I'm not a medical person, so keep it plain. Type of visit: [e.g. regular check-up / new problem / follow-up / specialist] Main reason I'm going: [e.g. "knee pain when I climb stairs"] How long it's been going on: [e.g. "about 3 weeks"] Medicines and supplements I take: [e.g. "blood pressure pill once a day, vitamin D"] What I'm most worried about: [e.g. "that it's arthritis" / "the cost of tests"] 1) Turn my notes into a one-page summary I can hand the doctor: symptoms, when they started, what helps or makes them worse, and my medicines. 2) Put my 3 most important questions at the top, worded clearly. 3) Add up to 5 good follow-up questions people often forget (what the next steps are, what to watch for, when to call back, costs or alternatives). 4) Keep it under one page. Don't diagnose me or tell me to change any medicine. If something in my notes sounds urgent, say so plainly at the top. My notes (name, birth date, and insurance numbers removed): [PASTE YOUR NOTES]

Done when: You have a one-page list with your top 3 questions first, it matches what you actually feel, and it’s printed or saved on your phone for the visit.

Make it better: After the visit, paste what the doctor said and ask “Explain this in plain words and list what I need to do this week, with dates.” Going with a parent? Ask “Rewrite this as a list I can bring for my mom’s appointment, written so she can read it herself.”

Privacy and caveats: AI is not a doctor and can’t examine you, so use this to organize your own words, not to diagnose or change treatment. Don’t paste your full name, birth date, Social Security, insurance member ID, or patient portal login. Check the medicine names and doses against your bottles. If you have chest pain, trouble breathing, signs of a stroke, or think it’s an emergency, call 911 instead of preparing a list.
